A new way to contact us online

Our New Online Consultation service from 15th March 2024

There is a new way to contact us online. It helps you access the help and advice you need at the right time, in the right place, with the right person.

If you need help with a non-urgent medical problem or an admin request, you can contact us online. The service is available on our website from 14th March 2024 and also from the NHS app or at the NHS website from April 2024.

How it works

If you have a non-urgent healthcare concern or or admin request, fill out the online form. It will be read by our team to decide the right care for you. We will respond to every online request within 2 workings days

How long is the form? Will it take me a long time?

The form asks for your symptoms, how long you have had them and whether they have changed. We ask how you would like the surgery to help you and how we can contact you, once we have reviewed your request.

I do not have a computer or smartphone. How can I fill out a form?

A friend, relative, or carer can complete this form on your behalf. Our reception staff may completing the form over the telephone for you. Your request will then go with the other patient requests we receive.

Can a friend, relative, or carer do the form for me?

Yes. On the form, there is a box where we ask if the form is being completed by somebody else. We will only be able to reply to the patient, unless there consent on your medical record to speak to someone else.

Here are some of the benefits of our new form:

  • Easy and simple to use.
  • No long triage questions.
  • You can track your request.
  • We can send back a link to book the best appointment.
  • You can self-refer to some local health services, without needing to see a doctor first.
  • Accessible in the NHS app and NHS website.
  • No need to create login if you do not want to.

If you do not want to download anything, you do not need to. You don't need to have a username and password to use our form.
