Shared Care Prescribing
A Shared Care Agreement is when a hospital specialist and the GP work together to prescribe certain medications.
- Starting Treatment: A specialist must start the medication and check it is working well for at least 3 months before asking the GP to continue prescribing.
- GP Prescribing: We can only prescribe if we have a signed Shared Care Agreement with the specialist.
- Regular Check-Ups: Patients must have health checks (e.g., blood pressure, weight) to keep getting prescriptions.
- Medication Changes: Only the specialist can change the dose or type of medication.
- If No Shared Care Agreement: The specialist must continue prescribing the medication.
